(FR) MÉTHODE D'IDENTIFICATION D'UNE ZONE CIBLE EXPLOITABLE CHIRURGICALEMENT DANS LE CERVEAU D'UN PATIENT ÉPILEPTIQUE
Abstract
(EN) The invention relates to a method of identifying a potentially surgically operable target zone in an epileptic patient's brain. According to the invention, the method comprises the following steps: providing a computerized platform modelling various zones of a primate brain and connectivity between said zones; providing a model of an epileptogenic zone and a model of the propagation of an epileptic discharge from an epileptic zone to a propagation zone; obtaining a patient's personalized computerized platform; deriving the potential target zones based on modularity analysis; evaluating the target zones' effectiveness by simulating epileptic seizures propagation in the personalized patient's computerized platform; evaluating the target zones' safety by simulating spatiotemporal brain activation patterns in a defined state condition and comparing the simulated spatiotemporal brain activation patterns obtained before removal of the target zone with the spatiotemporal brain activation patterns obtained after removal of the target zone; identifying the target zones which satisfy both effectiveness and safety evaluation criteria as potentially surgically operable target zones.
(FR) L'invention concerne une méthode d'identification d'une zone cible potentiellement exploitable chirurgicalement dans le cerveau d'un patient épileptique. Selon l'invention, la méthode comprend les étapes suivantes consistant : à fournir une plateforme informatisée modélisant diverses zones d'un cerveau de primate et une connectivité entre lesdites zones ; à fournir un modèle d'une zone épileptogène et un modèle de la propagation d'une décharge épileptique depuis une zone épileptique vers une zone de propagation ; à obtenir une plateforme informatisée personnalisée d'un patient ; à déduire les zones cibles potentielles sur la base d'une analyse de modularité ; à évaluer l'efficacité des zones cibles par simulation de la propagation des crises d'épilepsie dans la plateforme informatisée personnalisée du patient ; à évaluer la sécurité des zones cibles par simulation de motifs d'activation cérébrale spatio-temporels dans une condition d'état définie et comparaison des motifs d'activation cérébrale spatio-temporels simulés obtenus avant élimination de la zone cible avec les motifs d'activation cérébrale spatio-temporels obtenus après retrait de la zone cible ; à identifier les zones cibles qui satisfont aussi bien des critères d'évaluation d'efficacité que de sécurité comme des zones cibles potentiellement exploitables chirurgicalement.
