Electrophysiologic biomarkers for prognostication of neurological outcome are described herein. An inverse correlation was found between timing of a cortical spreading depolarization (SD) wave and neurological outcome as tested at 24 hours post-CPR. Additionally, a mirror image of this SD was identified as a "repolarization (RP) wave." Quantifying features of SD and RP during cardiac arrest and cardiopulmonary resuscitation (CPR) provide important metrics for diagnosis and prognosis of neurological injury from hypoxia-ischemia and can serve as an early prognostication tool for predicting outcome at subsequent days after successful CPR. This discovery may also allow for novel therapeutic interventions to improve neurological recovery after hypoxia-ischemia insults.

La présente invention concerne des biomarqueurs électrophysiologiques pour le pronostic d'un bilan neurologique. Une corrélation inverse a été identifiée entre la synchronisation d'une onde de dépolarisation par étalement (SD) corticale et un bilan neurologique tel que testé à 24 heures post-RCP. De plus, une image miroir de cette SD a été identifiée comme une "onde de repolarisation (RP)." La quantification des caractéristiques de SD et RP pendant l'arrêt cardiaque et la réanimation cardio-pulmonaire (RCP) fournit des mesures importantes pour le diagnostic et le pronostic d'une lésion neurologique provenant d'une hypoxie-ischémie et peut servir d'outil de pronostic précoce pour prédire l'issue des jours après une RCP réussie. Cette découverte peut également mener à de nouvelles interventions thérapeutiques pour améliorer la récupération neurologique après des agressions d'hypoxie-ischémie.
